Abstract
Accurate, clear and coherent documentation is a pre-requisite for ethical social work practice that contributes to social justice and anti-oppressive outcomes for individuals, families, groups and communities. This chapter highlights some of the issues, contexts and conflicting demands that impact the development of high-quality social work records which contribute to individual and family wellbeing.
