Abstract
A 46-year-old male Swiss traveller spent a 2-week holiday in the border region of Argentine and Brazil. Two weeks after his return, he developed flu-like symptoms and exertional dyspnoea. Ten days after the onset of symptoms, the patient presented at his general practitioner with fever and a pronounced frontal headache. Physical examination did not reveal any abnormalities. The differential blood count was unremarkable and C-reactive protein was only mildly elevated. After ruling out sinusitis by x-ray and dengue fever by serology, the provisional diagnosis of an unspecified viral infection was made.
Over the following 10 days, the patient developed a non-productive dry cough. Chest x-ray revealed an infiltrative process in the right upper lung field ( Fig. 101.1 ). A CT scan of the chest showed a 2 × 4 cm consolidation in the right upper lobe with ipsilateral mediastinal and hilar lymphadenopathy, a discrete right-sided pleural effusion and multiple pulmonary micronodules in both lungs ( Fig. 101.2 ) (100.0°F).
