Abstract
[Extract] In this issue of the European Journal of Vascular and Endovascular Surgery, Johannesson and colleagues report a retrospective analysis of prosthetic fitting rate and explore the relationship between transtibial incision technique, post-operative treatment, and time to prosthetic fitting using data from the Swedish Amputation and Prosthetics Registry and the Swedish National Inpatient Register.1 The authors found that 859 (35.2%) of the 2 443 transtibial amputations were fitted with a prosthesis. Prosthetic fitting rates were much lower for knee joint disarticulation and transfemoral amputations. Multivariable modelling suggested that the following factors were associated with longer time to prosthetic fitting: older compared with younger age; use of post-operative silicone compression or directly laminated socket treatment compared with other treatments; having a contralateral amputation; and undergoing an amputation in an acute setting. The authors suggest that the use of a sagittal incision technique allows the use of a silicone compression therapy post-operatively, which enables early prosthetic fitting. It is currently unknown whether this accelerates time to ambulation and increases rates of successful prosthesis use in the long term.
