Abstract

In this issue of the European Journal of Vascular and Endovascular Surgery, Ha and colleagues report a systematic review and meta-analysis examining the value of intravascular ultrasound (IVUS) in guiding endovascular treatment of femoropopliteal artery disease.1 IVUS guided coronary revascularisation is an established treatment, with a recent meta-analysis of 15 randomised controlled trials involving 9 354 patients finding that it leads to a lower risk of cardiac death (relative risk [RR] 0.49, 95% confidence interval [CI] 0.33 – 0.72) and reductions in the risk of major adverse cardiovascular events (RR 0.64, 95% CI 0.51 – 0.80), myocardial infarction (RR 0.74, 95% CI 0.59 – 0.94), stent thrombosis (RR 0.48, 95% CI 0.29 – 0.81), target lesion revascularisation (RR 0.60, 95% CI 0.48 – 0.75), and target vessel revascularisation (RR 0.54, 95% CI 0.43 – 0.69) compared with angiography guided coronary revascularisation.2 By contrast, the use of IVUS is less established in the treatment of peripheral artery disease.
