Abstract

Diabetes is a positive risk factor for cardiovascular events and therefore the negative association between diabetes and abdominal aortic aneurysm (AAA) has long been considered odd. In this issue of the European Journal of Vascular and Endovascular Surgery, Koo and colleagues report an analysis of the association between diabetes and the risk of AAA.1 In a retrospective study of the Korean national health screening programme, the investigators reported protection from AAA in people with diabetes increasing with duration and severity of impaired glucose control.1 Strengths of the study included the size of the population studied at more than four million Korean people, detailed adjustment for other risk factors, and the multiple sensitivity analyses performed. Limitations included that AAA diagnosis ascertainment was based on hospital data and, given the condition is usually asymptomatic, misclassification is likely to be common. Nevertheless, the study by Koo et al. confirms the findings from a large body of research indicating that diabetes is protective both against diagnosis and progression (growth and rupture) of AAA.2, 3, 4
