Abstract
[Extract] Background and Aim: A growing body of research suggests that evidence-based interventions may tackle high rates of premature hospital readmissions among discharged patients with decompensated cirrhosis.Care bundles are a prime example of an evidence-based intervention that can reduce hospital readmissions. Currently, readmission rates and mortality for patients with liver cirrhosis at Blacktown Hospital are high, prompting this review of current standards of care upon discharge.
