Abstract
In this chapter, the authors discuss strategies for the future as well as discussing the challenges of providing comprehensive paediatric surgical care in a smaller department. Aboriginal and Torres Strait Islander people experience inequalities in healthcare treatment and outcomes. Paediatric surgeons are perhaps the last of the general surgeons. Workforce planning has often used the blunt tool of waiting lists and patient numbers to determine the number of surgeons required to operate a paediatric surgical service, but population shifts can be rapid resulting in unmet need in areas that are growing rapidly. It is an essential part of good clinical practice to maintain and update skills. Despite the challenges to practice in remote and regional areas, it remains a hugely rewarding area to practice in. The rise of populations in the tropics will require a shift in skills and training to meet demand.
