Abstract

In order to address the shortage of specialist medical practitioners in Pacific Island countries and the consequent reliance on expatriate staff, a four year Masters program to train specialists in internal medicine, surgery, paediatrics, anaesthesia, or obstetrics and gynaecology commenced at the Fiji School of Medicine in 1998. The first year of the program for each discipline consists of a Diploma year, and each discipline has a program consisting of a course work component and a research project. There is also a strand of professional development through all four years and a public health strand common to all of the disciplines.

In internal medicine, the coursework component consists of 10 modules per year, each of 4 weeks duration, for the first three years. These modules cover the major subspecialities in internal medicine. The practical component consists of employment as a general medical registrar in the department of medicine of an approved teaching hospital, with approved consultant physician supervision. The psychomotor skills component consists of certification of competency in performing echocardiography and gastroscopy, as well as other diagnostic procedures of the student's choice. The research component consists of a module on performance of a project under appropriate supervision.

At the end of 2002, 14 students had successfully completed the Diploma program, three had successfully completed the Masters program, and five were in years two to four of the Masters program.
