Abstract
J-T-D-17-07244 Re: The life-saving effect of “adenosine, lidocaine, and magnesium” cocktail during hypovolemic shock: One stone,three birds?
We thank Xiang, Klemcke, and Ryan for their interpretation of our adenosine, lidocaine, and magnesium (ALM) data in rat and pig models of severe hypovolemic shock and for their comments on the potential use of ALM for the treatment of traumatic shock in prehospital or battlefield scenarios. In their analysis, Xiang and colleagues, however, did not mention that ALM also has a profound survival benefit in the rat model of polymicrobial sepsis after 6 days without antibiotics. Thus, unraveling the underlying mechanisms of ALM appears to apply to both hemorrhagicand pathogen-induced traumatic injury and shock. In the June issue of the Journal of Trauma and Acute Care Surgery, we further showed that ALM increases survival after moderate traumatic brain injury.
