Abstract

Ein 34-jähriger Mann äthiopischer Herkunft stellte sich mit seit fünf Monaten bestehenden entzündlichen lumbosakralen Rückenschmerzen vor. In der Bildgebung zeigten sich multifokale Osteolysen, histologisch entsprachen die Läsionen einer granulomatösen Entzündung. Bei initial negativer mikrobiologischer Diagnostik konnten multiresistente Mycobacteria tuberculosis (MDR-TB) erst im Verlauf nachgewiesen werden. Während der MDR-TB-Therapie entwickelte der Patient multiple Nebenwirkungen. Dieser Artikel fokussiert auf die Herausforderungen in der Diagnose und Therapie der MDR-TB auch in einem sehr gut ausgestatteten Gesundheitssystem wie der Schweiz.

A 34-year-old man from Ethiopia had been suffering from inflammatory lower back pain for five months. The imaging showed multiple osteolytic lesions histologically corresponding to a granulomatous inflammation. Detection of multiresistant Mycobacteria Tuberculosis (MDR-TB) was delayed because of initially negative microbiological findings. During the therapy of MDR-TB, the patient developed different side effects. In this article we focus on the challenge to diagnose and treat MDR-TB also in a resource-rich setting as it is the case for Switzerland.
