Abstract

This report has been prepared as a scoping document to identify the data needed to evaluate health and wellbeing initiatives delivered to three communities in the Cape York region of Queensland. The initiatives are to be delivered under a program known as the Integrated Family Centred Indigenous Responsive Care System (the Transformation Project). A pilot of the Transformation Project is being arranged by Cape York Health Services District (CYHSD) for the communities of: Mapoon, Napranum and Weipa (the study area).

One of the aims of this report was to identify evidence-based priority areas to help close the gap in life expectancy between Indigenous and non-Indigenous people in the study area. It is also thought that the priority areas will help inform the content of the Transformation Project as to the type of initiatives to be delivered to the study area. A further task of this report was to identify patterns of disease and the major risk factors characterising Indigenous people in the study area. It also aimed to suggest evidence-based indicators to measure progress of initiatives that target each the priority areas. A gap analysis was undertaken at key stages throughout the study to identify information that was not available but needed for a successful evaluation. Finally, the report aimed to provide an outline of an evaluation framework that could be used to evaluate the CYHSD Transformation Project.
