Abstract

[Extract] Extramedullary plasmacytomas may be clinically mistaken for soft tissue tumors. Fine needle aspiration cytology (FNAC), the first line of investigation, can provide a reliable diagnosis in such cases.1-5 We wish to highlight the role of FNAC in recognizing clinically unsuspected soft tissue plasmacytoma through four cases. The clinical features and findings on investigation are summarized in Tables I and II. Cytologically, smears from all four cases showed sheets of plasma cells and plasmablasts (Figures 1 and 2). Many binucleated and multinucleated atypical plasma cells were also noted. The features were unequivocal for a diagnosis of plasmacytoma, and additional investigations were advised. A bone marrow aspirate showed features of myeloma in the first two cases and reactive plasmacytosis in the third and was normal in the fourth. With the help of serum electrophoresis, radiologic and bone marrow examination, cases 1 and 2 were diagnosed as multiple myeloma and cases 3 and 4 as extramedullary plasmacytoma. The first patient subsequently developed a pleural effusion, smears from which showed atypical plasma cells and plasmablasts (Figure 3).
